Cardiovascular system and hyperthyroidism. Pathophysiological bases and its clinical manifestation

The aim of this review is to reinforce the mechanisms of action of thyroid hormone on the heart and peripheral vascular system, anatomic and structural changes that occur at myocardial level, interaction with the sympathetic nervous system, impaired electrical activity, hemodynamic changes on myocardial contractility and ventricular relaxation. The clinical manifestations of hyperthyroidism can lead to symptoms such as palpitations initial advance to a state of greater complexity such as heart failure (HF), an entity of high morbidity and mortality. Heart failure could be expressed with rhythm disturbances, damage and impaired ventricular function valve, which left to their natural evolution may lead to death of the patient, giving relevance to the early recognition by professionals of the hormonal etiology of HF and/or the cause of his decompensation. This diagnosis allows early treatment and effective as euthyroid states lead to the normalization of cardiovascular disease in most patients.

Depressive symptoms and hospitalizations for heart failure: prevalence, predictors and mortality

Studies of depressive symptoms (DS) has been made in patients hospitalized for heart failure (HF) and the influence of various characteristics has been ignored. A prospective study was conducted in order to assess prevalence, predictors and correlated to the severity of DS and mortality in HF hospitalizations. Sociodemographic and clinical data were analyzed and the main outcome was death. Authors concluded that DS are common in HF hospitalizations. Their prevalence varies according to clinical characteristics of patients. The predictors were gender, marital status, lifestyle and etiology of HF. Mortality was related to moderate and severe DS.

Epidemiology of pulmonary hypertension

As a topic of clinical medicine and medicine science, pulmonary hypertension (PH) has made great progresses in a relatively short time. Within past decades interest in pulmonary arterial hypertension has increased, being reflected in the large amount of publications about diagnosis, poor prognostic indicators, new drugs and knowledge of its pathogenesis. Unfortunately, there are no epidemiological data available in most countries, including ours that has sub-registry evidences. We must reinforce programs towards a better knowledge of this disease, and propel a unique data registry.

We present a case of female, 41 years old, who had cyanosis at rest, lower limb edema and atrial septal defect type communication interatrial diagnosed in echocardiography who was tackled drug with calcium channel blockers.
